Position Summary:
The Cardiopulmonary Rehabilitation Nurse is a Registered Nurse responsible for the assessment, planning, implementation, coordination, and evaluation of care for patients participating in cardiopulmonary rehabilitation and diagnostic Holter/stress testing. The nurse provides patient education, supervises therapeutic exercise, and monitors cardiac rhythms and physiological responses to ensure patient safety and optimal outcomes.
This position collaborates with physicians and the interdisciplinary rehabilitation team to develop and implement individualized treatment plans, provide patient and family education, and maintain timely documentation in accordance with organizational policies and regulatory standards. The Cardiopulmonary Rehabilitation Nurse coordinates with ancillary departments to support comprehensive services for adult and geriatric patients participating in the cardiopulmonary rehabilitation program.
Essential Functions:
- Completes accurate and timely documentation of pertinent patient information in the eMAR using the VersaCare system.
- Functions in compliance with regulatory standards established by IDPH, TJC, OSHA, and other applicable agencies.
- Communicates effectively with interdisciplinary services regarding patient needs as indicated.
- Demonstrates flexibility to meet patient care needs; cooperates to provide coverage when needed, cross-trains to other areas as required, and accepts assignments willingly.
- Completes the initial patient assessment and initiates the plan of care within 24 hours of the patient’s first visit.
- Develops and implements individualized patient care plans based on exercise tolerance and physician-prescribed exercise prescriptions.
- Updates patient care plans as changes in patient condition or treatment occur.
- Completes the initial 6MWT, treatment plans, consents, and required questionnaires; submits or faxes appropriate medical assessments and treatment plans to the ordering provider within 24 hours of the patient’s initial visit.
- Completes follow-up Individualized Treatment Plans (ITPs) every 30 days and distributes them to appropriate providers and departments.
- Educate patients on the prevention, recognition, and management of exercise-related injuries.
- Assesses patients before, during, and after exercise sessions and stress testing, making appropriate referrals and recommendations to patients, staff, and the attending physician or Medical Director when concerns are identified.
- Accurately recognizes changes on cardiac rhythm strips and takes appropriate action to ensure patient safety.
- Anticipates and responds appropriately to emergency situations, following established emergency protocols and chain of command.
- Identifies and communicates unusual or abnormal patient assessments or care concerns to the rehabilitation supervisor.
- Reviews instructions with patients, family members, and/or significant others prior to transfer or discharge and ensures appropriate documentation is sent with the patient and to the ordering provider.
- Monitors and documents vital signs, Rate of Perceived Exertion (RPE), Rate of Perceived Dyspnea (RPD), weight, physiological responses, and clinical observations for Phase II and Phase III cardiopulmonary rehabilitation patients.
- Provides recommendations and constructive suggestions to improve patient care and program quality.
- Develops and participates in patient education events, group exercise sessions, presentations, handouts, and educational materials for cardiopulmonary rehabilitation participants.
- Enters EKG orders into Athena and utilizes the Relay device to request EKG services when indicated.
- Ensures physician orders for cardiopulmonary rehabilitation, Holter monitors, stress tests, and tilt table tests include appropriate diagnoses and physician signatures.
- Ensures chart documentation and clinical interventions are completed accurately and within required timeframes.
- Identifies patient needs for home exercise guidelines and provides appropriate education and recommendations.
- Ensures physician orders for Holter monitors include diagnosis, monitoring duration, and physician signature.
- Sets up Holter and ZIO AT monitors, uploads data, and properly scans reports to prepare for physician interpretation.
- Submits diagnostic reports into Epiphany and assigns the appropriate provider for interpretation.
- Reports critical Holter findings to the ordering provider once the report has been confirmed per cardiology.
- Prepares and monitors patients during stress testing and tilt table testing, ensuring patient safety and proper documentation throughout the procedure.
- Demonstrates knowledge of and adheres to policies and procedures for Stress Echo, Cardiolyte, Lexiscan, standard exercise stress tests, and tilt table testing.
- Demonstrates proficiency in initiating intravenous (IV) access when required.
- Demonstrates knowledge of and adherence to medication policies and protocols for medications such as Atropine and Aminophylline.
- Adheres to the organization’s Code of Ethics and Corporate Compliance Program and complies with all applicable policies, procedures, regulatory requirements, and accreditation standards.
- Participates in the development and implementation of program services including SilverSneakers® classes, group exercise and education sessions, accelerated cardiopulmonary rehabilitation workshops/classes, and traditional cardiopulmonary rehabilitation programs.
- Provides instruction and supervision for SilverSneakers® and other group exercise classes according to patient ability, safety guidelines, and program protocols.
- Supports both accelerated and traditional cardiopulmonary rehabilitation programs, ensuring individualized patient progression, safety, and adherence to physician-prescribed exercise plans.
- Collaborate with the multidisciplinary cardiopulmonary rehabilitation team, including physicians, nurses, respiratory therapists, exercise physiologists, and rehabilitation staff, to coordinate comprehensive patient care.

Position Requirements:
· Graduate from an associate degree, baccalaureate degree (preferred).
· Current RN licensure in the state of Illinois.
· Minimum of three years’ experience preferred
· Current BLS, ACLS certification required. PALS (preferred).
Pay Rate:
Pay ranges from $26.688/hour to $42.740/hour (rate of pay is based on applicable years of experience)
